Many of our patients are covered by 'extended health' or corporate healthcare plans! At our centre, we work with you to go over your personal coverage and help you design a program that suits your budget and meets your unique healthcare needs. Extended Health Coverage is usually provided for some of all of the following services which we provide at our centre:
We invite you to book an appointment with our Sports Injury Director (Dr.Laura McDonnell) and let our staff know that you would like to go over your extended health plan for specific services! Making a Claim through your extended health plan:Most employers have a standard claim form based on which insurance company they are affiliated with. Please speak to your HR dept for access to this claim form either in paper or electronic format. After receiving an insured service which you may claim under your plan, you will be provided with an invoice receipt from our clinic for you to include alongside your claim form. This receipt will provide the clinic information, date of the insured service, the practicioner who treated you, the practicioner's liscence number and the charges that were paid. Your insurance company will re-imburse you the cost of the insured service according to your plan. |
